Mother in laws has a problem with her car.... Help!

When cold you can just about get gears, clutch pedal bite point at bottom of travel. When warm no chance of reverse or 1st, 2nd a bit crunchy. Also and probably related is that when you have got it in first gear if you accelerate it 'jumps out' of the gear into neutral. I had this happen to me once with a Mark 1 XR2 and it was due to a engine mounting pin being loose and causing engine movement and it was knocking it out of gear.

Any ideas ???

Can you hear the gears being selected? No whine, howl and graunching from the transmission?

Could be a bad clutch?
Worn Gears?
Worn clutch release bearing??
